Job Description

The Social Work Supervisor supervises a social work team to efficiently and effectively provide care to senior care participants.   The Social Work Supervisor oversees all activities and resources of the Social Work program.  The Social Work Supervisor ensures appropriate department staffing.  The Social Work Supervisor completes performance evaluations and maintains accountability for their work team. Is a member of the care coordination team, participating in the assessment, planning and follow-up of participant care and facilitates communication between participants, their families and the SeniorCare staff.  Provides the clinical supervision of any Associate Clinical Social Workers (ACSWs) and graduate social work interns as needed.  The Social Work Supervisor serves as a clinical consultant to social work team members, providing guidance and/or direct intervention in resolving challenging situations and high risk, problem-prone cases. The Social Work Supervisor may review, develop, and implement policies and procedures, in assuring the social work program's compliance with applicable internal and external regulations, and in the development and monitoring of quality assurance programs.  The Social Work Supervisor participates on government & community committees as assigned needed.  The Social Work Supervisor is responsible for identifying operational barriers to effective provision of services and seeking resolution, and for providing services in an efficient and cost-effective manner.
Responsible for the psychosocial assessment and care plan for SeniorCare participants:
  • Performs and documents initial psychosocial assessments of prospective participants. 
  • Performs and documents quarterly psychosocial assessments and updates written treatment plans based on the assessment. 
  • Is a member of the care coordination team and responsible for care plan goals and interventions relative to all psychosocial issues.
Provides direct clinical services, which may include individual and family counseling, crisis intervention, facilitating education and support groups and linkage with resources to patients, families, and staff.
  • Performs biopsychosocial assessments and treatment planning for patients and families.
  • Establishes rapport, communicates effectively and empowers patients/families with diverse cultural norms and spiritual values to maximize and/or develop productive coping mechanisms, behaviors and thought processes.
  • Establishes and periodically revises the treatment plan on the basis of comprehensive and ongoing biopsychosocial assessment of patient/family's concerns, strengths, needs, clinical impressions, and issues specific to spiritual and cultural values. 
  • Appropriately gathers data reflective of the patient's and family's developmental stages.  Effectively incorporates data including age-specific needs into assessment and treatment planning for patients of all ages.
  • Identifies and effectively deals with resistance and conflict in work with clients (including patients, families, healthcare team members and community representatives).  Appropriately intervenes to decrease tension and assist clients in evaluating non-productive behaviors, thought processes and coping mechanism
  • Interprets clinical findings, promptly communicates assessment and treatment plan updates, and provides effective consultation on patient care issues to health care team and agency representatives, adhering to confidentiality guidelines.
  • Responds quickly to requests, flexibly adapts to changing priorities, and keeps other departments informed of progress on patient referrals and other work activities to ensure efficient coordination of activities.
Acts as a participant advocate and liaison between the participant and various governmental and private agencies in order to maximize the participant's support network and obtain needed services:
  • Facilitates communication between participant, family and SeniorCare staff.
  • Facilitates communication between participant and various programs such as SSI, Medi-Cal, Medi-Care, DSS, Social Security, and insurance companies.
  • Assists the participant to obtain housing and eligibility for special programs, such as “low income credit.”
  • Maintains a cooperative, positive working relationship with appropriate community agencies serving seniors.
Provides consultation to the SeniorCare staff on psychosocial and case management issues and participates in community meetings related to psychosocial and community services issues:
  • In cooperation with the Center Manager, facilitates/supervises support groups to encourage peer support and expressions of concerns.
  • Interfaces with Sutter and community referral sources to assist in building census.
  • Develops and manages relationships with agencies providing services to participant.
  • Establishes and maintains liaison with various educational institutions (i.e., Division of Social work, California State University, Sacramento) with regard to the training of interns in medical social work, when possible.
Provides leadership for their clinical social work team and assists the Director of Social Work & Palliative Care with the daily operations of the Social Work program.
  • Ensures adequate social work coverage including staff scheduling, case assignment and productivity monitoring.
  • Recruits talented staff by following HR guidelines for position posting, interviewing, and reference checks.
  • Coaches, counsels, and conducts performance reviews in conjunction with the Director.
  • Ensures that clinical social workers provide services in accordance with hospital policy and procedures, laws, regulations and professional standards.
  • Develops and monitors the social work program quality and performance improvement projects. Promotes staff development to enhance skill and competency of staff.
  • Provides orientation and ongoing, mentoring of clinical social workers.
  • Trains staff in departmental record keeping and monitors compliance with program standards for patient records and statistics.
  • In collaboration with the Director, assesses staff for competency needs and provides education, case reviews, and documentation reviews as needed
  • Holds staff accountable for positive customer service.
Works with supervisor and SeniorCare Leadership regarding compliance, budget development and management, safety committee, infection control and employee relations:
  • Ensures that Social Services Department is in compliance with all applicable government regulations and contractual requirements.
  • Prepares department for surveys conducted by any licensing/regulatory agencies.
  • Provides information and education to staff to ensure substantial compliance with SeniorCare and PACE rules and regulations.
  • Assists the Center Manager in the development and management of the budget for Social Services Department.
  • Develops, revises, as needed, and implements policies and procedures for the Social Services Department.
  • Participates in Supervisors Meetings.
  • Adheres to Universal Criteria and Policies and Procedures for Sutter Health and Sutter SeniorCare.
Provides Clinical Case Consultation for ACSW's & LCSW's as Needed
  • Provides opportunities for case consultation during individual and group supervision appointments for ACSWs.
  • In conjunction with the social work leadership team, the LCSW Clinical Supervisor is available for on-going clinical case consultation for all clinical social workers.  Issues related to facility and/or program operations will be directed to the appropriate manager/supervisor.

 



Qualifications

Education:
  • Graduate of an accredited college or university with a Master's Degree in Social Work (MSW) required.
  • Licensed Clinical Social Worker (LCSW) required.
Experience / Knowledge / Skills:
  • At least three years experience in social work, some of which must have been experience with seniors in a clinical or direct service role. 
  • Two years experience supervising social workers (MSWs) preferred
  • Able to relate well with the frail elderly and their families and with SeniorCare staff. Practiced psychotherapy as part of their clinical experience for at least two years within the last five years immediately preceding the provision of clinical supervision.
  • Experience as a clinical social worker in a health care setting required.
  • Experience providing mental health services in an inpatient and/or outpatient setting preferred.
  • Experience with discharge planning and/or case management in an inpatient setting.
  • Experience with palliative care and the psychosocial needs of the patient and family at end-of-life.
  • Experience, training and education in providing clinical supervision for Associate Clinical Social Workers to include:
  • Familiarity with supervision literature
  • Facilitation of therapist-client and supervisor-therapist relationship
  • Evaluation and identification of problems in therapist-client and supervisor-therapist relationships,
  • Utilization of a wide range of supervisory modalities
  • Demonstrated ability to:
  • Complete biopsychosocial assessment
  • Diagnose per Diagnostic & Statistical Manual of Mental Disorders
  • Plan for intervention and therapy
  • Coordinate adjunctive resources

Special Skills/Equipment: 
  • Must be able to act calmly and with patience in all situations, especially sensitive situations involving difficult participants and/or families as well as emergency situations.
  • Must be flexible and able to deal well with schedule changes, staffing changes, staffing shortages and after hours call.
  • Requires creativity in the development and implementation of participant care plans and services.
