Licensed Vocational Nurse – (LVN, LPN)

Job Description

Snowline Hospice is a nonprofit, community-based organization dedicated to meeting the unique physical, emotional, and spiritual needs of those who are nearing the end of life's journey. Our goal is to enhance the living, comfort the dying, and support the grieving with compassion and dignity. Snowline cares for patients and families in Sacramento County and El Dorado County.

The right candidate will have experience following a plan of care, providing treatments, and working collaboratively with members of a team to help facilitate positive patient care outcomes.

Job Responsibilities

As a LVN, you will be responsible for providing patient care to patients in their place of residence. Additional responsibilities of LVN include: • Performs nursing treatment • Performs assessment of patient's emotional status and needs of the family/caregiver • Provides care needs healthcare education to patients and family • Monitor, record, and report symptoms or changes in patients' conditions • Maintain accurate and detailed healthcare records • Work collaboratively with members of the Inter-Disciplinary team

Job Requirements Successful candidate for this position will be a graduate of an accredited practical nurse or vocation nursing program and have a current LVN license with the State of California Additional requirements of the Licensed Vocational Nurse (LVN) include: • A minimum of one year professional nursing experience. Community health/hospice or medical/surgical experience is preferred • Demonstrated ability to efficiently operate a computer, electronic medical record (EMR) experience preferred • Current CPR certificate • Possession of a valid California driver's license, access to a reliable automobile and auto insurance
