Jobing Description
Primary Responsibilities:
Assists with personal hygiene, elimination, feeding and mobility according to the patient's needs and the nursing orders of the plan of care
Assists with medications that are ordinarily self-administered.
Provides support for and re-enforces the IDG team's teaching of the patient's caregivers.
Communicates with the hospice RN Case Manager regarding services provided and significant findings.
Documents the care provided and patient's response to care the day the care is provided.
Participates with the IDG team in the development and implementation of the patient's and family's plan of care.
Follows written patient care instructions prepares by the supervising Registered Nurse.
Provides hands-on personal care; assists in ambulation or exercise; performs simple procedures as an extension of therapy or nursing services.
Reports all changes in the patient's medical, nursing, rehabilitative, and social needs to the Registered Nurse, as the changes relate to the plan of care and quality assessment and improvement activities.
Completes patient documentation on the date the service is provided.
Completes initial and annual competency skills list observed by a Registered Nurse.
Completes a required 12 hours of in-service education annually or more when required by state regulations.
Assists patient / family in light housekeeping duties such as preparing a meal or changing bed linens.
Attends and participates regularly in IDG/Staff meetings.
Submits documented visit records and completes all job duties in a manner that meets the accepted standards of practice and the Evercare Hospice & Palliative Care Policies and Procedures.
Contributes to achieving the company's mission.
Works collaboratively with others to achieve goals.
Participates in the quality assessment and performance improvement program.
Participates in hospice in-service trainings.
Follows accepted standards of practice for effective infection control and safety.
Engages in on-going self-development plan to build personal leadership skills, organizational knowledge, and enhance knowledge of the industry.
Identifies and addresses skill and/or knowledge gaps to fulfill job responsibilities
Seeks opportunities to network with colleagues, share experiences, willingness to provide direct support, solicit feedback and exchange ideas.
Develops and achieves professional growth goals and objectives and reviews with superior.

Skills / Requirements
Requirements:
- High school diploma/GED required.
- Certified Home Health Aide Certification in the state of California
- Possess valid driver's license and insurance
- Ability to deal effectively with the demands of the job and the population served.
- Ability to work without consistent direct supervision.
- Demonstrates communication skills, including the ability to read, write, and verbally report clinical information to patients, care givers, and hospice staff
Assets:
- CHHA experience
- Hospice experience
